Navicent Health Continues Commitment to Navigate Patients Towards Wellness with New Program

In support of its commitment to navigate patients toward greater health and wellness, Navicent Health has launched OrCarestra, a new program to assist patients in the coordination of care.
 
Conductors with Navicent Health’s Access Navigation Center will orchestrate healthcare services for each individual patient in order to best meet their needs. OrCarestra conductors will assist healthcare consumers in the following ways:
 
·         By providing quick, free information about local health services and options.

·         By providing worry-free assistance locating a local physician or service to fit the patient’s individualized needs.

·         By scheduling necessary appointments, as well as follow up tests and services as needed.

·         By developing consumer-facing support tools, including patient itineraries for scheduled care.
 
In addition, OrCarestra’s care navigators, assigned to each of Navicent Health’s inpatient units, will work closely with physicians and other clinical staff to map out a clinical pathway for each patient. With the help of the centralized navigation team, patients will receive care in the appropriate clinical setting. Employees at Navicent Health’s Access Navigation Center will contact patients after discharge and assist with scheduling follow up appointments, diagnostic tests, therapies, and other health services that may be needed to complete care. These services will be scheduled close to the patient’s home whenever possible.
 
This new program not only provides easier access to care for healthcare consumers, but will also benefit physicians and hospitals across the state who refer patients to Navicent Health for care. Referring physicians and hospitals need only call one phone number to reach an OrCarestra conductor, who will begin the process of managing the patient’s care. Navicent Health will monitor major areas of cost across the continuum, including total costs, Medicare Spend per Beneficiary, and specific site of care costs for commercial payors.
